That's when a light-colored van pulled up and someone in the back seat opened fire, according to Chicago Police and the Cook County medical examiner’s office.</p> <p>Hughes was shot in the head and was taken to Stroger Hospital, where he was pronounced dead at 6 p.m. Sunday, according to the medical examiner’s office.
